New patients: Please arrive 15 minutes early to complete intake paperwork.
Bring a valid photo ID and insurance card (if applicable).
Bring a list of any current medications and supplements.
Wear comfortable clothing, especially for well-woman exams.
Have questions ready, we want to hear them all.

We accept Medicare. Medicaid enrollment in progress. Self-pay and sliding scale options available.
